Surveillance/ നിരീക്ഷണത്തിൽ നമുക്ക് പിഴവ് പറ്റി എന്നായിരുന്നു ഒരു കണ്ടെത്തൽ. രോഗാണുക്കളോട് ഉള്ള യുദ്ധം തന്നെ ആണ് പൊതു ജന ആരോഗ്യം. (ഈ ലേഖനം വായിക്കാൻ 15 മിനുട്ട് എടുക്കും.)</div><div><br /></div><div><b>രോഗനിരീക്ഷണവും രോഗനിര്ണായവും</b> :</div><div><br /></div><div><br /></div><div>ഇത് രണ്ടും രണ്ടാണ് . പക്ഷെ പരസ്പരം ബന്ധപ്പെട്ട് കിടക്കുന്നു. ഒരു രോഗി പനി, ചുമ യുമായി ഡോക്ടറുടെ അടുത്ത് വരുന്നു. അന്വേഷണത്തിൽ മൂന്നാഴ്ച ആയി ചുമ ഉണ്ട് എന്ന് കണ്ട് ഡോക്ടർ ക്ഷയ രോഗ പരിശോധന ക്ക് കഫം കൊടുക്കാൻ പറയുന്നു. ലാബിൽ പോയി രോഗി അത് ചെയുന്നു. റിസൾട്ട് വരുന്നു. ക്ഷയ രോഗം സ്ഥിരീകരിക്കുന്നു. ഡോക്ടർ മരുന്നിന് കുറിക്കുന്നു.</div><div>ഇത്രയും ആയാൽ രോഗനിർണയം കഴിഞ്ഞു. ( ലാബ് ടെസ്റ്റ് ചെയ്യാതെ നിർണയിക്കുന്ന രോഗങ്ങൾ ഉണ്ട് എന്നത് മറക്കുന്നില്ല )</div><div>ഇതിന് ശേഷം രോഗിയുടെ വിവരങ്ങൾ തൊട്ടടുത്ത സർക്കാർ ആരോഗ്യകേന്ദ്രതിലേക്കോ മറ്റൊരു സർക്കാർ ഉദ്യോഗസ്ഥനെയോ അറിയിക്കുന്നു.ഇത് രോഗനിരീക്ഷണവും ആയി.</div><div>രോഗലക്ഷണങ്ങൾ ഉള്ള ആളെ ഒരു ലാബ് പരിശോധന ക്ക് വിധേയമാക്കി രോഗം ഉണ്ട് അല്ലെങ്കിൽ ഇല്ല എന്ന് ഉറപ്പു വരുത്തുക എന്നതാണ് രോഗനിരീക്ഷണം അഥവാ Surveillance .</div><div>പൊതുവെ ,നിരീക്ഷണം ഏർപ്പെടുത്തിയിട്ടുള്ള രോഗങ്ങളിൽ ഡോക്ടറുടെ ലാബ് പരിശോധനക്ക് മുൻപുള്ള രോഗനിർണയത്തിന് പ്രസക്തി ഇല്ല. അതായത് മേൽപറഞ്ഞ ഉദാഹരണത്തിൽ മറ്റ് പല ലക്ഷണങ്ങൾ കൊണ്ട് ഇത് ക്ഷയം അല്ല എന്ന് ഡോക്ടർക്ക് തോന്നിയാലും ക്ഷയത്തിന് ആയുള്ള കഫ പരിശോധന അധികൃതർ നിഷ്കര്ഷിക്കും. </div><div><b><br /></b></div><div><b>രോഗനിരീക്ഷണത്തിൽ ലാബ് ടെസ്റ്റ് കളുടെ പ്രസക്തി:</b></div><div><br /></div><div>പണ്ടൊക്കെ തളർവാതം വന്നവരുടെ കണക്ക് എടുത്ത് അതിൽ നിശ്ചിത ശതമാനം 'പോളിയോ കൊണ്ട് ആയിരിക്കും' എന്ന് അനുമാനിക്കുക ആണ് പതിവ്. എന്നാൽ ഇന്ന് ,കയ്യോ കാലോ തളർന്ന കുട്ടികളിൽ നിന്നും സാമ്പിൾ ( മലം) ശേഖരിച്ച് വൈറസ് കൾച്ചർ ചെയ്ത ശേഷം ആണ് ഉണ്ട് അല്ലെങ്കിൽ ഇല്ല എന്ന് സ്ഥിരീകരിക്കുന്നത്.അതായത് കൃത്യത കൂടികൊണ്ടിരിക്കുന്നു. </div><div><br /></div><div>രോഗപകർച്ച യുടെ വിവിധ ഘട്ടങ്ങളിൽ ലാബ് ടെസ്റ്റിന്റെ പ്രസക്തി:</div><div><br /></div><div>ഒരു രോഗം ആദ്യമായി ഒരു സമൂഹത്തിൽ എത്തിപ്പെടുമ്പോൾ രോഗം എവിടെ, ആരിൽ,എപ്പോൾ ഉണ്ടാവുന്നു എന്ന് അറിയാൻ ടെസ്റ്റുകൾ ചെയ്യണം. താരതമ്യേന രോഗത്തിന്റെ തോത് കുറഞ്ഞിരിക്കുന്ന ഈ സമയത്തു കൂടുതൽ ടെസ്റ്റ് ആവശ്യം ആയിവരും. രോഗലക്ഷണങ്ങൾ ഇല്ലാത്തവരെയും ചിലപ്പോൾ ടെസ്റ്റ് ചെയ്യും.</div><div>പിന്നീട് രോഗത്തിന്റെ പകർച്ച വർധിക്കുമ്പോൾ രോഗലക്ഷണങ്ങൾ ഉള്ളവർ എല്ലാവരെയും ടെസ്റ്റ് ചെയ്യും. ഇല്ലാത്തവരെ ചെയ്തു എന്ന് വരില്ല. എന്നാൽ രോഗനിർമാർജനം ലക്ഷ്യം ആണെങ്കിൽ ടെസ്റ്റുകൾ കൂടുതൽ വേണ്ടി വരും.അപ്പോഴും ചെറുതായി എങ്കിലും രോഗലക്ഷണങ്ങൾ ഉള്ളവരെ തന്നെ ആണ് ടെസ്റ്റ് ചെയ്യുക.</div><div>ഇനി, രോഗം തീരെ ഇല്ലാതാവുന്ന സമയത്തു അത് തെളിയിക്കാൻ രോഗനിരീക്ഷണതിന്റെ ഭാഗമായി ടെസ്റ്റ് ചെയ്യണം. ഉദാഹരണത്തിന് </div><div>"ഞങ്ങൾ മൂന്നാഴ്ച ചുമ ഉള്ള 2000 പേരെ ടെസ്റ്റ് ചെയ്തു. ആർക്കും ക്ഷയം ഇല്ല.അതിന്റെ അർത്ഥം ഈ പ്രദേശത്ത് നിന്നും ക്ഷയം തത്കാലം അപ്രത്യക്ഷമായി എന്നാണ്".</div><div>അതായത് രോഗപകർച്ച യുടെ വിവിധ ഘട്ടങ്ങളിൽ വിവിധതരം ലക്ഷ്യങ്ങൾ ആണ് ടെസ്റ്റിങ്ങിന് ഉള്ളത്.</div><div><br /></div><div>കോവിഡും ടെസ്റ്റും:</div><div><br /></div><div>കോവിഡിന്റെ കാര്യത്തിൽ തുടക്കം മുതൽ തന്നെ വൈറസിന്റെ കണികകൾ കണ്ടു പിടിക്കുന്ന RTPCR ആണ് ടെസ്റ്റ് ആയി ഉപയോഗിക്കുന്നത്. ഇത് ജീവനുള്ള വൈറ്സിനെയും ജീവൻ ഇല്ലാത്ത വൈറസിനെയും കണ്ടു പിടിക്കും.രോഗാണുക്കൾ ശരീരത്തിൽ പ്രവേശിച്ചു ഏതാനും ദിവസം കഴിഞ്ഞാൽ രോഗലക്ഷണങ്ങൾ കാണും . അത് വരെ ഉള്ള സമയത്തിന് ആണ് ഇന്കയുബഷൻ കാലം എന്ന പറയുന്നത്.ഇതിന് മുൻപ് തന്നെ രോഗി മറ്റുള്ളവർക്ക് രോഗം പകർന്ന് കൊടുക്കുന്ന സ്ഥിതിയിൽ ആവുന്നു എന്നതാണ് കോവിഡിൽ ഒരു പ്രശ്നം. ഇത് കോവിഡിന്റെ മാത്രം പ്രത്യേകത ഒന്നും അല്ല.ശരാശരി 200 പേർക്ക് പോളിയോ വൈറസ് ബാധ ഉണ്ടായാൽ ഒരാൾക്ക് ആണ് തളർവാതം വരിക. അല്ലാത്തവരുടെ മലം വഴി അസുഖം പടർന്ന് കൊണ്ടേ ഇരിക്കും.</div><div><br /></div><div>പക്ഷെ ടെസ്റ്റ് പോസിറ്റീവ് ആയില്ലെങ്കിൽ രോഗപകർച്ച ഉണ്ടാവുമോ? ഇവിടെ നാം ടെസ്റ്റിന്റെ പരിമിതികൾ അറിയണം.</div><div><br /></div><div><b>ഉള്ളതെല്ലാം പറയുന്ന ടെസ്റ്റുകളും ഇല്ലാത്തത് പറയാത്ത ടെസ്റ്റുകളും:</b></div><div><br /></div><div>ശാസ്ത്രീയമായി ഇതിന് സെൻസിറ്റീവിറ്റി ,സ്പെസിഫിസിറ്റി എന്ന് പറയും.</div><div><br /></div><div><i>ഉള്ളതെല്ലാം പറയുന്ന ടെസ്റ്റുകൾ </i>: </div><div><br /></div><div>ഇവ വളരെ സെൻസിറ്റീവ് ആണ്. കുഴപ്പം എന്താണെന്ന് വെച്ചാൽ ചിലപ്പോൾ സത്യം അല്ലാത്തതും പറയും. രോഗം ഉണ്ടെങ്കിൽ അറിഞ്ഞിരിക്കും. പക്ഷെ ഉള്ളതെല്ലാം പറയുന്ന കൂട്ടത്തിൽ ഇല്ലാത്തത് ചുമ്മാ തട്ടി വിടും. ഇക്കൂട്ടർ ( ഈ ടെസ്റ്റുകൾ) ഇല്ല എന്ന് പറഞ്ഞാൽ ഇല്ല. പക്ഷെ ഉണ്ട് എന്ന് പറഞ്ഞാൽ ഉറപ്പിക്കാൻ വയ്യ.</div><div><br /></div><div><i>ഇല്ലാത്തത് പറയാത്ത ടെസ്റ്റുകൾ :</i></div><div><br /></div><div>ഇവ സത്യസന്ധർ ആണ്. പക്ഷെ പഴയ നമ്പൂതിരിയുടെ കഥ പോലെ മർമ്മം നോക്കി നോക്കി കാര്യം മറക്കും. അതായത് പറഞ്ഞാൽ ( ടെസ്റ്റ് പോസിറ്റീവ് ആയാൽ ) വിശ്വസിക്കാം.കോടതി ഭാഷയിൽ പറഞ്ഞാൽ സത്യമേ പറയൂ. പക്ഷെ സത്യമായിട്ടുള്ളതെല്ലാം പറയണം എന്നില്ല. </div><div>ഇതിൽ നിന്നും ഒരു കാര്യം വ്യക്തം. രോഗനിർണയം നടത്തുന്നതിന് ആദ്യം "ഉള്ളതെല്ലാം പറയുന്ന "ടെസ്റ്റ് ചെയ്യുക.അത് പോസിറ്റീവ് ആയാൽ ഇല്ലാത്തത് പറയാത്ത ടെസ്റ്റ് ചെയ്യുക. ഇപ്പോൾ പോസിറ്റീവ് ആയാൽ രോഗം ഉറപ്പ്.</div><div>കോവിഡിന്റെ കാര്യത്തിൽ "ഉള്ളതെല്ലാം പറയുന്ന ടെസ്റ്റ് "ലഭ്യമായിട്ടില്ല. </div><div><br /></div><div>ഇപ്പോൾ ഏതാണ്ട് 70 ശതമാനം മാത്രമാണ് സ്രവം പോസിറ്റീവ് ആവുന്നത്. അതായത് പിന്നീട് നടന്ന സ്രവം ടെസ്റ്റ് വഴി രോഗം ഉണ്ട് എന്ന് കണ്ടവരിൽ ആദ്യത്തെ ടെസ്റ്റ് 30ഓളം ശതമാനത്തിൽ നെഗറ്റീവ് ആയിരുന്നു.</div><div>അപ്പോൾ ശ്രദ്ധിക്കുക. കോവിഡിന്റെ rtpcr ടെസ്റ്റ് പോസിറ്റീവ് ആയി വന്നാൽ അത് ഉള്ള കാര്യം ആണ്. കാരണം Specific ആണ്. ഇപ്പോൾ ഞാൻ പറഞ്ഞ ഭാഷയിൽ "ഇല്ലാത്തത് പറയാത്ത" ടെസ്റ്റ് ആണ്. </div><div>വളരെ അപൂർവമായി സാങ്കേതിക കാരണങ്ങളാൽ ഉണ്ടാകാം.പക്ഷെ പ്രായോഗികമായി ഇത് കണക്കാക്കേണ്ടത് ഇല്ല.</div><div><br /></div><div><br /></div><div>എന്തായാലും "ഉള്ളത് എല്ലാം പറയാത്ത.".. സെൻസിറ്റീവ് അല്ലാത്ത ,ടെസ്റ്റ് ആയത് കൊണ്ട് തന്നെ എത്ര കൂടുതൽ ആളുകളിൽ ടെസ്റ്റ് ചെയ്യുന്നോ അത്രയും രോഗ നിരീക്ഷണം മെച്ചപ്പെട്ടതാവും.</div><div><br /></div><div>(ഇതിന്റെ മറുവശം പറഞ്ഞാൽ "ഉള്ളത് ഒക്കെ വിളിച്ചു പറയുന്ന" ടെസ്റ്റുകൾ വളരെ അധികം നടത്തിയാൽ ഉള്ളതിൽ കൂടുതൽ കേസ് ഉള്ളതായി റിപ്പോർട് വരും.ഇത് പരിഭ്രാന്തി സൃഷ്ടിക്കും.</div><div><i>കോവിഡിന്റെ RTPCR സ്രവ പരിശോധന കാര്യത്തിൽ ഈ പ്രശ്നം ഇല്ല.)</i></div><div><br /></div><div><b>ആരെ ടെസ്റ്റ് ചെയ്യും</b>?</div><div><br /></div><div>അസുഖങ്ങളുടെ രോഗനിര്ണയത്തിന് വേണ്ടി ആണെങ്കിൽ ആരെ ടെസ്റ്റ് ചെയ്യണം എന്ന കാര്യത്തിൽ വലിയ ആശയക്കുഴപ്പം ഒന്നും ഇല്ല.രോഗലക്ഷണങ്ങൾ ഉള്ള ആളുകളെ ടെസ്റ്റിന് വിധേയമാക്കുന്നു.മിക്കവാറും ശക്തമായ സംശയം ഉണ്ടെങ്കിൽ ആണ് ഇത് നടക്കുക.എന്നാൽ രോഗനിരീക്ഷണത്തിന്റെ ആവശ്യത്തിന്, അത്ര സംശയം ഇല്ലെങ്കിലും ടെസ്റ്റ് ചെയ്യാൻ പൊതുജനാരോഗ്യ വിഭാഗം ഡോക്ടർമാർക്ക് നിർദേശം നൽകിയേക്കും.</div><div><br /></div><div>ഒരു ചെറിയ ഉദാഹരണം പോളിയോ നിരീക്ഷണത്തിൽ നിന്നു തന്നെ.</div><div><br /></div><div>കുട്ടികൾക്ക് പല കാരണങ്ങൾ കൊണ്ടും കൈകാലുകൾക്ക് തളർച്ച അനുഭവപ്പെടാം.പോളിയോ അതിൽ ഒന്നു മാത്രം. ഇപ്പോൾ ആണെങ്കിൽ ഇന്ത്യയിൽ പോളിയോ വൈറസ് കണ്ടിട്ട് വർഷങ്ങളായി. പക്ഷെ ഇന്നും പെട്ടെന്ന് കൈകാലുകൾ തളരുന്ന കുട്ടികൾക്ക് പോളിയോ വൈറസ് ടെസ്റ്റ് നിർബന്ധമാണ്. ഇത് പറയുമ്പോൾ പല ഡോക്ടർമാരും എന്നോട് തർക്കിക്കും. ഞങ്ങൾക്ക് അറിയാം. ഇത് പോളിയോ അല്ല. പിന്നെ എന്തിന് ടെസ്റ്റ് ചെയ്യണം?. ഒറ്റ ഉത്തരം രോഗനിരീക്ഷണത്തിന് ഇത് ആവശ്യം ആണ് എന്നതാണ്.</div><div><br /></div><div>പോളിയോയുടെ കാര്യത്തിൽ ആ വൈറസ് നമ്മുടെ ഇടയിൽ ഇല്ല എന്ന് തെളിയിക്കാൻ കൂടി ആണ് ടെസ്റ്റ്. </div><div><br /></div><div><br /></div><div>അപ്പോൾ കൃത്യമായ രോഗലക്ഷണങ്ങൾ ഇല്ല എങ്കിലും.. രോഗപകർച്ചാസാധ്യത ഇല്ല എങ്കിലും ( contact അല്ല എങ്കിലും) രോഗനിരീക്ഷണം( surveillance) കാര്യക്ഷമമായി നടക്കണമെങ്കിൽ ടെസ്റ്റ് നടക്കണം.</div><div>അപ്പോൾ ന്യായമായ ഒരു ചോദ്യം ഉയരും. ഭൂരിഭാഗം രോഗികൾക്കും ലക്ഷണം ഇല്ല. അപ്പോൾ പിന്നെ എല്ലാവരെയും ടെസ്റ്റ് ചെയ്യണോ?.</div><div><br /></div><div>അത് പ്രായോഗികമല്ല. ഒരു കാര്യം ഓർക്കുക. എന്തെങ്കിലും രോഗലക്ഷണങ്ങൾ ഉള്ളവരിൽ ആണ് ഒന്നും ഇല്ലാത്തവരെക്കാൾ വൈറസ് സാന്നിധ്യത്തിന് സാധ്യത.അത് കൊണ്ട് കോവിഡിന് സമാനമായ ലക്ഷണങ്ങൾ ഉള്ളവർക്ക് എല്ലാം RTPCR ടെസ്റ്റ് ചെയ്യണം. </div><div>ഇതിന് SARI, ILI ഇങ്ങനെ രണ്ട് തരം ലക്ഷണ ശാസ്ത്രം ഉപയോഗിക്കാം.</div><div><br /></div><div>Influenza Like Illness : കഴിഞ്ഞ 10 ദിവസത്തിന്റെ ഉള്ളിൽ 38 ഡിഗ്രിക്ക് മുകളിൽ പനി, ചുമ. </div><div>Severe Acute Respiratory Illness ( SARI ) :</div><div>കഴിഞ്ഞ 10 ദിവസത്തിന്റെ ഉള്ളിൽ 38 ഡിഗ്രിക്ക് മുകളിൽ പനി, ചുമ. കൂടാതെ ആശുപത്രിയിൽ പ്രവേശനം. </div><div><br /></div><div>കേരളത്തിൽ ഇപ്പോൾ ചെയ്യേണ്ടത് ഇതാണ്. ഇത് കുറെ ദിവസം മുൻപേ ഞാൻ ഇംഗ്ലീഷിൽ എഴുതിയിരുന്നു. ലിങ്ക് കാണുക.</div><div><br /></div><div><b>ലക്ഷണങ്ങൾ ഇല്ലാത്ത രോഗികൾ </b>:</div><div><br /></div><div>ഇവരെ കണ്ടുപിടിക്കാൻ എളുപ്പം അല്ല.അപ്പോൾ ചെയ്യാവുന്ന കാര്യം രോഗം സ്ഥിരീകരിച്ചവരുടെ രോഗലക്ഷണങ്ങൾ ഇല്ലാത്ത സമ്പർക്കക്കാരെ ( contact) ടെസ്റ്റ് ചെയ്യുക എന്നതാണ്. ഇത് മേല്പറഞ്ഞ SARI/ ILI ലക്ഷണശാസ്ത്രം ഉള്ളവരുടെ കോണ്ടക്ടസിനും ബാധകം ആക്കുക. രോഗികളുടെ ടെസ്റ്റ് റിസൾറ്റിന് കാത്തു നില്കേണ്ടതില്ല. കാരണം നേരെത്തെ പറഞ്ഞ ടെസ്റ്റ് "ഉള്ളതെല്ലാം പറയുന്ന "ടെസ്റ്റ് അല്ലാത്തത് കൊണ്ട്.</div><div><br /></div><div><b>ഒടുവിൽ ഒരാൾ മാത്രം ബാക്കിയായി :</b></div><div><br /></div><div>പലരും ചോദിക്കുന്നു. ഇത് എപ്പോൾ തീരും?. വാക്സിനോ മരുന്നോ ഇല്ലാതെ വന്നാൽ അനന്തമായി നീളുമോ? ഇല്ല. ഇവിടെയും രോഗനിരീക്ഷണം നമ്മുടെ രക്ഷക്ക് എത്തും. ടെസ്റ്റ് പോസിറ്റീവ് ആവുന്ന ഒരാൾ ഏകാന്തവാസത്തിൽ..ഐസോലാഷനിൽ പ്രവേശിക്കുന്നതോടെ ആ സംക്രമണ ശൃംഖല...transmission chain അവിടെ മുറിഞ്ഞു. ഇങ്ങിനെ പരമാവധി കേസ് കണ്ട് പിടിച്ച് സംക്രമണ ശൃംഖലകൾ മുറിക്കണം.</div><div><br /></div><div>ഒരു ഘട്ടത്തിൽ ടെസ്റ്റ് ചെയ്യുന്നവരുടെ എണ്ണം ദിവസം 5000വും പോസിറ്റീവ് 30 എന്നോ മറ്റോ വരും. ഇത് നല്ല രീതിയിൽ ഐസോലാഷൻ ചെയ്യുന്ന പക്ഷം പതുക്കെ ചെറിയ സംഖ്യ യിലേക്ക് ചുരുങ്ങും. </div><div>ഈ ഘട്ടത്തിൽ നമ്മൾ കോവിധാനന്തര സമയത്തേക്ക് എത്തും. പിന്നെയും കേസ്/ ക്ലസ്റ്റർ വരും</div><div>അതിന് പ്രാദേശികമായ നിരോധനാജ്ഞ, ഐസോലാഷൻ മറ്റും മതിയാവും.</div><div><br /></div><div>Test.. The medical college is a strange place. Anybody who’s not studying there would like to have studied there. But all those who are studying there wish they were not there . The first year student is faced with an avalanche of information in the form of Anatomy of the human body ,it’s metabolic functions in Physiology and its chemistry in Biochemistry. Anatomy is not half as interesting as made out by connoisseurs of six pack and size zero. It’s all dead and frozen with the soulful accompaniment of the powerful stench of formalin. Many a wannabe doctor would faint in the first day. Yours truly did not, because I never thought of it as human body. It was all leathery and dry. Physiology in our times used to be full of carbon coated drums and croaking poor frogs who were butchered for study. Maneka Gandhi put paid to that and all frogs can breath easy. But the pain of pricking oneself for umpteen blood studies is heart breaking. Not to speak of the feeling when your early college crushes prick themselves. Biochemistry is your only link to the lab as we know from school days . Although collecting and testing urine is not exactly what you thought the guys in white overalls do. Phew.....<br />

Once you enter your third..now second year you’re led in a flock, to the Hospital. Lest you get any ideas ,the junior students are treated with just about the same respect as a doormat. They wander around the wards like headless chicken as one of our leaders once remarked. Once in a while a junior faculty takes pity on them and call them out. When we were third years ,we had a Profesor..let’s call him Zach...who was very sympathetic to us. In the sense he was very reluctant to take classes. But he would lead us suddenly to a patients bed side. The harassed patient would invariably be a cardiac patient . With an additional sound in their heart called a murmur. Zach was fond of these murmurs. So he would put his stethoscope chest piece to the patients chest and disengage the earpiece. He would then invite the students one by one to adorn them and listen to the heart sounds and murmur. He was so confident about our abilities that he never allowed us to place the steth ourselves.<br />

If you ever thought that doctors would hear amazing stuff through their tubes ,you will be cured by third year of Medicine. That year you will actually that accept doctors are divine because all that you hear will be hush push and lub dub. Zach would be describing a “ pan systolic murmur with a diastolic flow murmur.” You’re even more shocked your classmates, especially the female ones hear them.Later only you realise that all that they were hearing was the cloth rubbing on the chest piece. A Physician..those guys with an MD in General Medicine who perpetually detest the Cardiologists ...once remarked. “ Cardiologists are Gods. Because they can hear things other people can’t hear.” I would second that. It is another matter that the Echo machine deflated many of their egos. I’ve a friend who disagreed with the examiner about a murmur in a PG exam. The examiner was livid and demanded an Echocardiogram on the spot. It was done and lo and behold ,my friend ,the candidate was right. Just as surely he failed that attempt. Such are stuff Gods are made of.<br />

Coming back to bedside ,the only worthwhile stuff junior students do is to study history taking. This is a laborious process . In the first days it takes an hour. I’ve always wondered what’s the point, since what we get in an OPD is all but five minutes . But it’s the custom and we need to follow it. The patient is subjected to a volley of questions starting from his childhood . By the time he is finished he’ll be too tired to speak and the student would be breathless. But some people, especially older ones enjoy the interaction.<br />

The examination part is hesitant and clumsy when it comes to junior years and the patient smiles through the process . There are experienced patients who tell students “my liver is enlarged 3 cm”. Especially in exams ,these kind of patients are a treasure. Some Examiners are smarter. They instruct the patients not to reveal a certain finding that’s not very obvious. Like a swelling in a private part . The fate of the student is sealed unless the patient takes pity on the junior doctor fumbling around the bedside.<br />

Once the examination is done, in the training period ,one sacrificial goat should present the case to the teacher . This is your training for the exams too..which you realise quite late. Most of us used to avoid this ,pinning hopes on an over eager student . And hoping that your turn never comes . One of my classmates made his turn memorable by stating that the patient has a Past history of suicide. The word "attempt "was swallowed by him with more swiftness than the guy once did his poison. The teaching Professor gave a quiziccal look and then burst out laughing. The class followed as you’re expected to,in a medical school.<br />

Then there was this guy who was quizzing a teenage girl for symptoms of thyroid disease. The guidebook said 'ask for intolerance to cold ',so he was keen.<br />
“ Do you feel cold without it being cold?” Was the first question.<br />
The girl said No.( obviously )<br />
He tried again.<br />
<br />
“ Do you feel abnormally cold ?”. The girl giggled but said no.<br />
He wasn’t one to give up easily.<br />
“ When it’s raining ...in the night..do you feel cold?”<br />
The girl went red probably from the imagery of a rainy night coming from the mouth of the guy was a handsome hunk. And she giggled and said “ Yes’<br />
<br />
He promptly wrote “intolerance to cold “ in the notes.<br />
<br />
Yours truly has had his share of bloopers. One that I remember vividly is searching the Obstetric ward once for a patient named prominently in the front page of case sheet as Rajalakshmi. I reported back to the Staff Nurse that I couldn’t find the patient. She gently pointed out that Rajalakshmi was the name of the treating doctor and not the patient .<br />
<br />
The cake goes to the hapless female student who was appearing for her Microbiology exam. She was a chronic additional in Medical college parlance ,meaning she was a repeater many times over. The internal examiner wanted her to pass . The external examiner..the dreaded species in any medical course exam who lands from another university...helpfully agreed to ask a simple question.<br />
“ Can you name a single celled organism?”<br />
The internal suddenly realised the candidate was confused and he stealthily gestured towards the wall where an enlarged picture of amoeba was hanging . As luck would have it another creature of a different species chose the exact time to cross the picture. So our candidate confidently answered.<br />
“ Cockroach,Sir”<br />
It is not known what happened to the examiners but such are the stuff legends are surely made of.</div>

The Man who knew Infinity<br />

It's tough to make biopics, tougher to be authentic and still tougher to make it engaging ,especially when your subject dabbled with higher mathematics. The Man Who Knew Infinity directed by Mathew Brown based on the life of Mathematical genius Srinivasa Ramanujam pulls off this task with elan. The story of this Indian Mathemetician who was honoured with a Royal Society Fellowship is well known. We already had one biopic in Tamil on him titled Ramanujam. I haven't seen it so can't offer any comparisons. The current movie portrays his life,work and passion with taste and honesty. Dev Patel acquits himself well in the title role while Jeremy Irons lives his role as Prof. Hardy.Devika Bhise puts in a spirited performance as Janaki,the wife of the genius. The life and times of the great Indian is brought out well by the arts department and the England scenario which is most of the movie ,is also perfect to a fault. The highlight of the movie is that the subject of Mathematics is dealt with seriously ,not dumbed down. The scene where Hardy explains the " partitions " is an example and his comment " even you can understand this " is almost directed at the audience.<br />
Today education is seen as a part of one's job application. A necessary evil that has to be suffered before one settles for a job and a high salary. Employability is a buzzword and the pursuit of knowledge for the sake of it is considered archaic. This movie is a reminder that pursuit of pure science is a pleasure which should not be denied to our gennext in the guise of making them employable. Science for the sake of science is part of what makes man great. Let's keep it that way at least for the gifted.<br />
For starters how many of us know the Chennai houses two world class Mathematical institutes?</div>

One of the first things that you learn as you train as a Child Specialist is that the dose of medicines you prescribe is crucially dependent on the weight of the child. Nowadays for many drugs body surface area is used. It is more accurate but difficult to use since you need a ready reckoner. The reckoner will give you surface area from the weight and height of the child. When I started my Paediatric practice therefore the weighing machine was an important instrument that I purchased. By experience you learn to guess the weight from age and appearance especially in busy out-patient departments. But when you have just started to practice you are more scientific .<br />
So when this young woman and ( apparently) her mother walked into my OPD with a toddler hanging on to their hands I understood there was trouble in store. Most parents make their young children stand on the weighing machine ,without realising that they might start crying when left alone. Simple alternative is to weigh both parent and child while the parent carries them,and then subtract the weight of the parent.<br />
When my examination of the child was over , I turned to them and said<br />
" We need to check his weight "<br />
The young woman promptly tried to make the already apprehensive three year old stand on the machine . He promptly started to whimper. Before his whimper turned into a cry ,I told the older woman<br />
" You hold the child. We can take the mothers weight first. Then we can take their weight together and find out his weight"<br />
The young woman hesitated and said<br />
" But doctor ,there is a problem "<br />
" What? " I asked .<br />
" I am not his mother " she said solemnly.<br />
I almost fell off the chair laughing ....</div>

It was in his forties that my father decided to pursue specialisation in Child Health. When he started practising Medicine there was no need for specialisation to make a decent living. The general practitioner was well respected and derived a lot of job satisfaction. But as years rolled on, the need to specialise increased and there were lots of peer and job pressure on GPs to go in for specialisation . Being in the Government Health services gave my father the opportunity to pursue specialisation under special rules for In-Service candidates. Admission to this "Service quota " was strictly according to seniority and therefore he was 45 by the time he completed Paediatrics PG course. When he started specialty practice he however continued to see older patients as well. This story is about a hilarious incident in Paediatric practice. It is very important to be able to examine a calm child to make a correct diagnosis . Most Paediatric texts advise that extremely young infants be examined in the lying position. However most toddlers become restless when forced to lie down,so this age group that is from about 8 or 9 months till about five years the children are best examined while they are seated in the mother's or father's lap. Many parents have the habit of placing their two or three year old in the examining chair in front of the Doctor and stand behind them. This is a sure recipe for the child to scream and make the whole interaction unpleasant. So when one afternoon a young mother made the same mistake ,my father gently chided her.<br />
" Please ," he said " keep him on the lap".<br />
" No doctor," the young mother hesitated " it will be inconvenient "<br />
" What ? " he asked incredulously " to keep a child in lap is inconvenient ? "<br />
With a lot of hesitation ,the young mother placed her one year old on the lap ...<br />
...of the doctor!<br />
My father did not know whether to laugh or cry ,but the infant knew. He cried and before his mother finally understood the mistake,had made the doctors lap his urinal.<br />

My father used to say about the incident as an example of how communicating correctly to patients is important .</div>

In the days when my father started practising House Calls were an important duty of the general practitioner. For the new generation let me explain that this was when the doctor went to the house of the patient and examined him or her. There were no ambulances then in rural areas and bed ridden patients used to get care by these House Calls. For the new practitioner this also represented an opportunity at better payment because house calls were always better paid. The caregivers who wanted house calls always waited with a taxi till the last patient in the doctors queue was seen. These visits also strengthened the bond between the doctor and the people. Often this meant wading through dirt and walking through fields but at the end it gave a satisfaction that's difficult to recreate in the current scenario . The essential equipments for this visit like stethoscope, BP apparatus and some medicines were always kept ready in a small briefcase earmarked for this purpose. What I about to talk about is an interesting incident in one of these house visits. My father used to be called frequently to see an octogenarian grandmother. Let's call her Mary for convenience. She was the typical devout Christian lady always clad in the chatta and mundu. While as family doctor my father was familiar with most families it was near impossible to be in the know of everything . So house visits were one opportunity to banter about the events in the family like marriages . So it happened that one evening he was called in again to see Mrs. Mary at her house. After finishing his examination and prescribing medicines he started some chat to humour the old lady.<br />
" So,how is your elder daughter doing? " he asked " she's in Irinjalakkuda,no ? "<br />
Mrs. Mary looked at him and nonchalantly asked<br />
" So you did not know it doctor ? "<br />
My father was perplexed since he did not know what he was supposed to know.<br />
He nodded " No".<br />
" She died two months back doctor "the octogenarian replied stoically.<br />
My father was shocked and said<br />
" I am so sorry to hear that, " he said " you must be feeling terrible "<br />
Her reply was instantaneous.<br />
" Oh no,was she not old doctor? She was past sixty "she replied and went back to her rosary counting ..</div>

Movies always begin with acknowledgements ,so let me also start by acknowledging that these write ups have been prompted by writings of my friend Srinivasa Raghavan about his legal career. But I had written some of these earlier but it did not occur to me to post it in FB. Some of this is my own,but most experiences are my father's and shared with us when he was alive.<br />

My father Dr Rajagopal moved into Ollur then a sleepy village near a sleepier town,Trichur ,Kerala in 1970,the year I was born. He was then in his late twenties ,full of zest and the aspiration to succeed. He joined the ESI dispensary in Ollur as its Medical Officer in charge . For the uninitiated,ESI stands for Employees State Insurance , a government entity providing health services to the organised labour class. Although he was a fresh hand ,my father was given charge specially by the then Director of ESI Dr. Chitran Nambudirippad apparently to end bickering between two existing doctors. When he landed up in the village ,he was provided a quarter behind the dispensary. Years later I joined government service in the very same dispensary. I went around the quarter where I had probably crawled around. It was by then a dilapidated wreck.<br />
Anyhow even then the quarter had its share of surprises,commonest of which were poisonous snakes! But this is not about any of those crawling creatures!<br />
This story is about the two doctors who were then practising. I shall refrain from naming them correctly since one is very much alive. They lived and practised on two nearby streets. They were the only doctors then and competition was tough. Fees were minimal at two or five rupees ,so your survival depended on the number of patients .<br />
The way they tried to maximise their clientele was rather stunning. They kept two youngsters at the start of the streets where they practised. Let's call them Dr Samuel and Dr John. When a family start walking towards Dr Johns clinic cum house,the youngster employed by his rival will come forward<br />
" Hey you are going to Dr John? Do you know he gives all the wrong medicines ? You will get cured only slowly. Turn back and go to Dr Samuel. Now, there is a fine doctor."<br />
At this if they turn back ,at the corner,the other youngster will accost them,pretending to be an innocent passer by.<br />
" Going to see Samuel Dr,?...do you really value the life of your child or......"<br />
Imagine the plight of the poor family ! There were also smarter ones who escaped paying fees with one doctor by pouring abuse on the other.<br />
When my dad started to practise,first thing he did was to befriend one of his competitors. The other one had already left.<br />
They remained friends till my fathers death in 2012. Their camaraderie despite competition was legendary and locals found it initially difficult to believe. Indeed many of my friends in school believed they were adversarial. Even though renowned as the land of reformers such as Chanakya , Buddha and like, Bihar is undoubtedly the most under developed State in India. Be it by the rampant political anarchy or by the curse of incessant floods, Bihar raises eye brows at those who seek a tad of development there. Dr.Santhosh Rajagopal depicts the vignette of one of Bihar’s remote village where he had a mission as a delegate from W.H.O.<br />
First written 2006<br />
The Baghmathi river was flowing surreptitiously quiet, as she had never been in spate, as if the fog that enveloped her early in the morning was perhaps the only thing dangerous about her. Her vast fertile banks, enveloping villages of Chandauli, Ganeshpur and numerous other hamlets teeming with people were separated from her fury by just a sand bund. It was about 7 in the morning and fog was omnipresent. The temperature was near freezing and no amount of warm clothes would prevent even the bravest from shivering. A convoy of vehicles appeared out of the fog. They have traveled atop the bund taking dangerous turns and swerves that sent the occupants on a macabre roller coaster ride. I was riding the first vehicle whose driver seemed to take a sadistic pleasure in rocketing through the most improbable of roads.<br />
On a mission<br />
We were in Bihar to help out in the Polio Eradication Drive, then hopefully in its last legs. The officer sitting behind me had been there for barely two months, but seemed to have taken in even the local dialect. The driver informed us that when the river was in spate, kilometers of water would be the only sight there. The bund then became a Noah’s ark, keeping the villagers alive till the river spent out her fury completely.<br />
The vehicles drew up at the depot where the vaccines were then ready for distribution. One by one, the vaccinator teams took delivery of their quota of the Oral Polio Vaccine and set off on their rounds ,on foot, deep into the riverine wilderness.<br />
A touch and go<br />
Some hours passed and it was time to monitor the activity of the teams. We were eight doctors and an equal number of Volunteers. My area was a bit in the interior, informed my guide. I hitched a ride on a bike of one of the volunteers. As it meandered its way up the slope of the bund, one bike preceding us got caught in the quick sand and crashed-, luckily ,no injury. We traveled on the bund which was roughly twenty feet wide. In many places people were living on it, which made it even narrower. Their buffaloes mowed and bleated as we played trapeze between them and the waiting river.<br />
We asked around for directions, occasionally consulting the map ,and the gospel of the eradication Drive – the Micro plan. The microplan lists all human habitations in a given area.While in Bihar it is as good as a Google map.<br />
The bike jumped down from hedges, and raced through slush, as I held on for precious life. Most of the time after descending the bund, we were traveling through backyards and fields. Roads, on which the toughest of off -roaders would have a fit, were made every year, my Volunteer informed me. Every year around June, the river makes mincemeat of them. The numerous islands of treacherous river sand on the roads testified to the correctness of his statement. After about 8 kms of that ride ,we came to a small school. We parked the bike and began hunting for the vaccinator teams.<br />
We passed a few houses covered by them already and checked their work. There were no electric poles, no telephone lines, the only link with civilization as we know it were the three bands on my cell phone which told me the nation’s oldest telecom operator was around.(Thank God for that).<br />
We met Arvind, a supervisor. He was about 50 and lightly built, that morning he had set out on a cycle with a vaccine carrier in search of his teams, and bumped into the babus. (us).He wanted to learn from how we worked, he informed us cheerfully. We wanted to see his teams, which were working about 4 km away. The path had water bodies and slushy areas, so walking was the only option. After a bumpy bone rattling ride, I was only too eager to accept that .We waded through the fields and walked atop bridges made solely of decaying vegetation. All around were the hinterland of our great country and I was benumbed by the primitiveness of it all. I took out my cell and called home. I cheerfully informed my wife that I was calling from the middle of nowhere.<br />
Heart of India<br />
We met a team made up of an elderly gentleman and a kid barely out of his teens. The lad was holding geru (a type of ink), to mark the visited houses; the senior was going about giving drops to children. Children were everywhere, dressed in nothing more than rags, eating out of full but flea infested plates, crowding into single room huts set one within the other, riding the omnipresent buffaloes. I remembered a remark of one of my colleagues about a place “swarming with kids”. The team was vaccinating the kids, marking their fingers and houses as well as managing formats, which had increased in number that time around. I watched ,as the old gentleman, obviously semi-literate, fumbled with the papers. We went around colonies of what the supervisor calls the lowest caste in Bihar. These are the Mushahars- literally, ones who eat rats. Later I learnt that the female literacy among them was 0.1%.There should be no castes, I meekly suggested, he agreed readily but went on about why they were the underdogs.<br />
<br />
As walked along he cautioned me on dangers of walking in the fields, and I politely informed him that I too came from a village and was no stranger to walking on fields and narrow bridges made of felled coconut tree trunks back home. He insisted no village can be as backward as his, and I reluctantly agreed. It was 4 PM and I suggested we could have a tea. Arvind promptly disappeared into a rather better looking house and reappeared with ginger tea of the best quality. He was the local Compounder, he informed me and the respect showered on him as we passed made it clear that he was more close to being a doctor in the locality. I had no illusions about any one of my professional brethren setting foot there, in what the Mahatma would have called the heart of India. Somehow during my entire stay I kept remembering the Mahatma, might be because Champaran, where he began his “career” of Satyagrahas was close by.<br />
Back through the dark <br />
It was about 5 PM and time to wind up. Tired from the long walk, Arvind offered us seats in another courtyard. As we settled down he waxed eloquent on how things could never change there. I strongly disagreed, and said it can be changed, provided we aspired to. Which suddenly made me think, what were all those kids aspiring to? They seemed to be contented with riding buffaloes into the fields, hang around with gur made from sugarcane, and tearing up and playing with posters of the just concluded elections. No entrance exams for them, no scholarships, no pencils or sharpeners, no schoolbags either. – Just the predictable grind of a rural farming life, with the most primitive of implements.<br />
We were offered Dahi,(curd) and I accepted. I refused the big cup, settling for half as much, only to regret later. I remembered we had skipped lunch, which explained the nectar like taste of home-made dahi.<br />
As the sun was quickly disappearing into the enveloping fog, we made haste and I dreaded the prospect of return through the route. Luckily we could find an alternate, slightly less dangerous path. As we came in, the rest of the team was getting a bit worried about us. As I approached, my local colleague asked me how the activity was. The dahi was elixir like, I told him as we bundled on to the waiting 4 wheel drives on our way back.<br />
As we rode into the darkness, I remembered an argument I had with my brother-in-law working for software major in Bangalore; on how much time India would take to become a developed nation. Ten, he had said. Fifty, I had wagered, fresh from a similar trip to rural UP. I called him up on my cell. “I have changed my mind on that”, I informed him. It would be hundred years…..” .The connection broke off, as if to reinforce the statement.<br />
POST SCRIPT:<br />
After several rounds of deployment to the capital Patna and the national capital,I finally revisited rural Bihar in 2010.By this time the government had changed .The first sign of better tidings as I entered a rural Primary Health Centre in Madhubani district was the drone of a generator that was ensuring 24 hour electricity supply.I was in for yet another surprise- at 7 PM a delivery was happening in the PHC! This was unthinkable in Bihar where some PHCs resembled cattle sheds once.I was told by a visibly irritated doctor that the villagers have given up home delivery since the “money” came. He was referring to the Janani Suraksha Yojana which gave a sum of money for hospital deliveries .I told him we should be happy since they can have safe deliveries. He looked at me and made a statement that has me speechless. ”Sir, if some complications happen at home and the mother dies, at least we doctors will not get the blame. Now we get blamed for everything.”<br />
We are so happy to blame politicians for everything that goes wrong. As I returned to my place of residence- a PWD guest house still with no electricity or water- I reflected on this statement .Nothing came to my mind except the proverb-“You can take a horse to water…….”</div>

தெருவுக்கு இருபுறமும் பள்ளியில் குழந்தைகள் நிற்பது போல் அக்ரஹாரத்து வீடுகள். கோவிலை நோக்கி மெல்லமாக நடந்து கொண்டிருந்தேன் . தானே வரைத்த சிவப்பு ஓவியத்தின் அழகை ரசித்து கொண்டு மாடியேறுவதரக்கு சோம்பலாக இருந்தது ஞாயிறு.<br />
கோபுரத்தை நெருங்க ஒரு அம்பாசிடர் கார் என் பார்வைக்கு வந்தது. எங்கேயோ பார்த்த ஞாபகம்,காரையும் அதில் உடகாந்திருந்த முகத்தையும். என்னை பார்க்கவும் அந்த முகம் மடிச்சார் புடவயும் வைரத்தோடுமாக கமலாமாமியின் உருவத்தில் அவதரித்தார்.<br />
"எப்ப வந்தாய் ?" புன்னகையுடனான கேள்வி.<br />
"இன்னைக்கு காலேலெ தான் மாமி," என்று நான் சொல்ல,மாமி திரும்பவும் கேட்க்கிறாள்.<br />
"ஆத்தில யாரிருக்கா?"<br />
தேவையற்ற கேள்வி.பல வருடங்களுக்கு முன் போன அம்மா,ஓரிரு வருடங்களுக்கு முன் மரைந்த அப்பா. தொலைவில் இருக்கும் நகரத்திலிருந்து விடுமுறைக்கு மட்டும் கிராமத்துக்கும் வீட்டிற்கும் வந்து எட்டி பார்ககிறவன் நான் என்பது மாமிக்கும் தெரிந்தது தான்.<br />

" யாரும் இல்லை" பதில் சொல்ல மாமி அடுத்த கேள்வி எழுப்பினார் .<br />

"அப்ப சாப்பாடெல்லாம் எப்படி?"<br />
<br />
"அதான் நம்ம அய்யர் ஹோட்டல் இருக்கே" என்றேன் நான்.<br />

"ஏன் ,எங்காத்துக்கு வரலாமே," மாமி சொன்னாள்."அங்க நானும் மாமாவும் மட்டும் தானே இருக்கோம்"<br />
<br />
ஒரு முப்பது வருடங்களுக்கு முன் எனில் ,மனதை குளிர வைத்திருக்கும்,அந்த அழைப்பு.பாதி மூடியிருக்கும் கதவின் மறைவில் இருந்து புறப்படும் ராதிகாவின் கொலுசின் ஒலி.அம்மாவின் கேள்விகளுக்கு நான் பதில் சொல்ல சொல்ல,அடக்கமாக மொழியும் மழை போல அவளுடைய மூச்சு சத்தம்...யாருக்கும் தெரியாமல் கடந்த வருடம் கோவில் தேர் திருவிழா அன்று நான் கொடுத்த வளைகளின் கலகலப்பு...<br />
<br />
"வேண்டாம் மாமி, இன்னக்கு ஒரு கல்யாண அழைப்பு இருக்கே.."என்று சொன்னேன் நான்.<br />
<br />
வெளிநாட்டில் கணவருடனும் மகிழச்சியுடனும் வாழ்ந்து கொண்டிருக்கும் ராதிகாவை வெளியில் நிற்கவைத்து கோவிலுக்குள் நுழைந்தேன் .<br />

மாலை இரவாகும் பொழுது வாசல் தீண்டாவிட்டால்<br />
ஆவலுடன் காத்திருக்கவும் வந்து விட்டால் கட்டி அணைக்கவும்<br />
செல்லம் கொடுத்து மகிழவும் ..<br />

എൺപതുകളുടെ മധ്യത്തിലെ ഒരു സായാഹ്നം.സ്ഥലം,ഞങളുടെ സ്ഥിരം സമ്മേളനവേദിയായ അഛന്റെയും അമ്മയുടെയും കിടപ്പുമുറി.അഛനു രോഗികളുടെ തിരക്കു കുറവുള്ള ഒരു നവംബർ മാസസന്ധ്യ.സമ്മേളനാംഗങൾ ഞങ്ങൾ കുട്ടികൾ,പന്ത്രണ്ടും പതിനാലും വയസ്സും പ്രായം,അഛൻ,അമ്മ...<br />
സംസാരം ഭൂമിക്കു കീഴെ എന്തിനെ പറ്റിയും ആവാം.കറണ്ട് ബില്ല്,ഇന്ദിരാഗാന്ധി,ജനതാപാർട്ടി,ആറാട്ടുപുഴ പൂരം,എടക്കുന്നി വിളക്ക്..പറഞ്ഞ് പറഞ്ഞ് സംസാരം കല്യാണം,പ്രേമലേഖനം ഇങ്ങനെ പോയി.അപ്പോഴാണു ഞങ്ങൾ കുസ്രുതികൾക്കു ഒരു സംശയം...(ആഴ്ച തോറും മുറ തെറ്റാതെ രാഗത്തിലും രാംദാസിലും കണ്ടുകൂട്ടിയിരുന്ന പ്രേം നസീർ സിനിമകളുടെ സ്വാധീനം ആവണം)...കല്യാണത്തിനു മുൻപ് അഛനും അമ്മയും പരസ്പരം കത്തെഴുതിയിരുന്നോ? പ്രേമലേഖനമാണു മനസ്സിൽ..സ്വതവേ നിറമുള്ള അമ്മയുടെ മുഖം കൂടുതൽ ചുവന്നു.<br />
"കൊത്തി കൊത്തി മുറത്തിൽ കയറി കൊത്തുന്നോ ?"<br />
ഒരു നാടൻ പ്രയോഗത്തിലൂടെ അമ്മ രക്ഷപ്പെടാൻ ശ്രമിച്ചു.ഞങ്ങളുണ്ടോ വിടുന്നു? സ്വതവേ തുറന്ന പ്രകൃതക്കാരനായ അഛനിലായി സമ്മർദ്ദം.അവസാനം അച്ഛൻ വഴങ്ങി.<br />
"പ്രേമലേഖനം എന്ന് പറയാനാവില്ല,എനിക്ക് ഒരു പാട്ട് എഴുതി അയച്ച് തന്നു.അക്കാലത്തെ പ്രസിദ്ധമായ ഒരു പാട്ട്"<br />
"ഏതാ,ഏതാ..." ഞങ്ങൾ തിടുക്കം കൂട്ടി.അച്ഛൻ മറുപടി പറയും മുൻപ് അമ്മ ആ ഗാനം പതുക്കെ മൂളി.<br />
"ഒരു കൊച്ചു സ്വപ്നത്തിൻ ചിറകുമായവിടത്തെ അരികിൽ ഞാൻ ഇപ്പ്പോൾ വന്നെങ്കിൽ...."<br />
വർഷങ്ങൾ കടന്നു പോയി.അമ്മ 1991ൽ തന്നെ ചിറകു വിരിച്ച് പറന്നു പോയി.പലപ്പോഴും ബെഡ് റൂം കോൺഫറൻസുകൾക്ക് ഞാനും അച്ഛനും മാത്രമാവും.2012 മെയ് മാസം ഞങ്ങൾ ആ ഞെട്ടിക്കുന്ന വാർത്ത അറിഞ്ഞു.അച്ഛനു കാൻസർ ബാധ.അധികം ആയുസ്സില്ല.ചികിൽസിക്കാൻ പോലും അദ്ധേഹം കൂട്ടാക്കിയില്ല. അല്ലെങ്കിലും ചികിത്സ കൊണ്ട് വലിയ ഫലം ഇല്ലായിരുന്നു എന്നത് സത്യം.<br />
ജൂൺ ആദ്യം എന്റെ ജ്യേഷ്ടൻ പറഞ്ഞതനുസരിച്ച് പത്ത് ദിവസത്തെ അവധിക്കെഴുതി കൊടുത്ത് ഞാൻ നാട്ടിലേക്ക് പുറപ്പെട്ടു. പുറപ്പെടും മുമ്പെ ഞാൻ അച്ഛനോട് പറഞ്ഞു.<br />
"സമ്മതമാനെങ്കിൽ ചികിത്സിക്കാം,അല്ലെങ്കിൽ നമുക്ക് വല്ല പഴയ കാര്യങ്ങളും പറഞ്ഞിരിക്കാം"<br />
നാട്ടിലെതിയപ്പോൾ ചികിത്സിക്കാൻ ഡോക്ടർക്ക് പോലും സമ്മതമല്ലായിരുന്നു. തീരെ ക്ഷീണിതനായി കിടന്നിരുന്ന ആശുപത്രി കിടക്കയിലായിരുന്നു ഞങ്ങൾ മൂന്നു പേരുടെ അവസാന"സമ്മേളനങൾ".മരിക്കുന്നതിനു അര മണിക്കൂർ മുമ്പുവരെ!<br />
അങ്ങനെ കിടക്കുംബോൾ കാൻസർ ചവച്ച് തുപ്പിയ അച്ചന്റെ മെലിഞ്ഞ കാൽപ്പാദങ്ങൾ ഞാൻ നോക്കിയിരിക്കും. അപ്പോഴെല്ലാം അമ്മ മൂളിയ,വർഷങ്ങൾക്കു മുമ്പ് വരികൾ കടമെടുത്ത് തന്റെ ഭാവി ഭർത്താവിനു അയച്ച് കൊടുത്ത ആ പാട്ടിലെ വേറെ ചില വരികൾ ആണു എന്റെ മനസ്സിൽ ഓടി നടന്നുകൊണ്ടിരുന്നത്.<br />
"പട്ടു പോലുള്ളൊരാ പാദങ്ങൾ രണ്ടും കെട്ടി പിടിച്ചൊന്ന് പൊട്ടിക്കരയാൻ..."<br />
ഇന്നിപ്പൊൾ മറ്റൊരു ലോകത്ത് അവർ ഒന്നിച്ചിരിക്കുന്നു.അല്ലെങ്കിൽ അങ്ങനെ വിശ്വസിക്കാൻ ആണു ഞങ്ങൾക്കിഷ്ടം.<br />
മരണത്തിനു ഏതാനും നിമിഷം മുമ്പ് ,അച്ഛനു നൽകിയ മരുന്ന് തുണിയിൽ പൊതിഞ്ഞ ഒരു ഡ്രിപ്പ് ആയിരുന്നു. പ്രകാശം തട്ടാതിരിക്കാൻ ചില മരുന്നുകൾക്ക് അങ്ങിനെ ചെയ്യാറുണ്ട്.അത് കണ്ടപ്പോൾ ഞാനും അച്ഛനും ഒന്നിച്ച് പറഞ്ഞ തമാശ ഞങ്ങൾ തൃശ്ശൂർക്കാർക്ക് മാത്രമേ മനസ്സിലാവൂ.<br />
"ഇതെന്താ കണിമംഗലം ശാസ്താവോ?"<br />

I have been moved by news reports have turned my face away from some, refused to read a few, but no news story, none ,have reduced me to tears .It happened today as I read the centre page write up in the Hindu by Reem Wasay about the barbaric macabre slaughter of innocence in Peshawar,Pakistan. For the first real time, ”words are insufficient “ is not a cliché. Indeed since the news broke I have been raving and ranting as many middle class armchair activist, about the choice of punishment the perpetrators should have got had they been captured alive. Yes, because death for them has been absolution, almost a reward that they so insanely crave for and get at such cost to humanity.<br />
Perhaps it was the fact that the children who became cannon fodder in the sub continental Armageddon for religious domination were the same age as mine. Perhaps it was the fact that we are all too human. However the last thing I would do is to describe this act as inhuman. Because No animal I repeat NO animal will kill with such impunity such callous barbarity. It is left to us humans to find new words ,indeed new lexicons to describe our civilization or the lack of it ,circa 2015.<br />
Every day as my children get ready for school, I help out in small measures-filling their water bottles with lukewarm water, mixing their tea or coffee, getting the breakfast from the kitchen prepared by their loving mother. Indeed these scenes would have been repeated endlessly in the hundreds of houses of those children who have now returned for ever. How does anyone make any sense of this horror? How does anybody reach this stage of paranoia which drives you to kill? How does one react to this as a father, a mother, a teacher?<br />
My first reaction as I said was that of any ordinary human. Get those bas**** responsible for this and inflict the most vicious punishment on them. Do not let them die and even appear to their mad followers to have attained jannath.<br />
But as ordinary human beings we have more to do than rave and rant. As we settle down back (as we all will) to our funny commercials on TV and funnier ones on our mobiles, let us not forget the blighted spring of Peshawar. Let us first think that the society we live in so used to violent methods of protest and persuasion that only the most macabre of them will shake us. We have come to believe that might is right. We think it alright to hold towns and cities to ransom in the name of sundry demands, destroy pubic property at will, kill people in the name of caste ,pride, religion, what not. We shrug and move on. Some even subliminally justify some of these. May be it is time to stop this violent discourse which has perniciously permeated the society.<br />
As an ardent admirer of Gandhiji I often wonder what he would have said in response to this kind of unspeakable horrors. Would he have still advocated non-violence? I do not have the answers, because I am not privy to the great soul’s thoughts. But as for a mere mortal like me, I would like to believe that my holding on (dearly ) to the principle that more violence is not the answer, comes out of a deep conviction and not weakness .<br />
We may not like it, but the honour killers of children, the child traffickers of the subcontinent ,the employers who abuse child labourers are all removed from the depraved Taliban only by degrees. By our tolerance of these violent impositions of faith, pride and prejudice ,we are condoning violence and teaching our children might is right. Gandhiji might be old fashioned but he was right in not allowing violence to win. He was once asked whether he will change an opinion in the face of imminent death.<br />
“No” he said “in that case they will have my dead body, not my opinion”<br />
The corridors of tomorrow’s spring in Peshawar have been lined with dead bodies, each one an attempt to make us believe that violence pays. We should not heed that macabre call. We should continue to believe that humanity is still around, that the next stranger that you see would open the door for you and not shoot at you. They can have all the dead bodies that they want. They will not have my opinion on the basic goodness of goodness itself. Period.</div>

He frequents my office with a stainless steel tumbler full of chundal –a healthy Tamil snack made of groundnut and coconut scrapings. He is good looking with a polite tongue that could be the envy of a diplomat. My office secretary used to buy from him sometimes because she was hungry and sometimes just because he was nice. I occasionally talked to him when he told me he is doing this to augment family income. His father was dead, mother was chronically sick and bed ridden. He had two elder sisters who stopped schooling in middle school and are now working in a beauty parlor .Today I had just returned from a lecture to doctors undergoing training in the Training Institute with which we share our office space. This was a good batch which interacted a lot and gave a polite applause at the end. So I was in a happy mood when I walked in and saw Muruganandam .I asked him how he was. He smiled and said he was okay. I could not see his Chundal and it was odd time –school hours. He smilingly informed me he had stopped attending regular school. He had also stopped the Chundal business. He had got a job in a factory making some mementoes. His mother and uncles had told him in no uncertain terms that he was expected to work and not waste time studying. He was in 8<sup>th</sup>.Now his kindhearted teacher was taking a weekend special class for him and marking him present on all days. Most of us adults would recollect at least one instance when on hindsight we would have”tried more hard” or taken a different route” or made a different choice. When the grades arrive all that we have is hindsight. In essence even this is learning. But in the more academic sense of the word, learning is the process of acquiring knowledge. Since basic knowledge in all fields is deemed essential in today’s world we have structured curricula in all subjects taught in schools. But it needs to be emphasized that mastering all subjects is not essential for success in life. Not knowing intricate details of historical events for instance may not be relevant for an Engineer but a sense of history is relevant if tomorrow he is become an IAS officer. Therefore learning is not mere accumulation of data but using new knowledge to leverage one’s innate instinct to succeed in life and equally importantly to become a socially responsible citizen. Passing an exam is a small event in this journey .Honestly how many of us recall our marks in the Quarterly examinations of Class 8? But right now, that seems to be the end of the road for your 13 year old!</div>

Retrieval of data is dependent on many variables but data that is linked to other data and then stored is easily retrieved .Let us look at memory itself-there are different kinds of memory-the short term one that helps you to dial a number moments after you heard it ,the medium term memory that helps you to write exams ,the long term ones that makes you recollect the past including childhood.</div>
